Who we are

Here at Georgetown Living Home Health and Companions, we are passionate about the seniors in our community. We believe they deserve the best of care so they may be able to live their best lives at home for as long as possible and that is what we provide. We are in constant contact with our patients’ physicians and work hard to coordinate with them along with the patients and their families, as well as with the other services that may be needed, such as pharmacies to provide medications, our own outpatient therapy, Staywell Therapy, and also, our own hospice, Tranquility Hospice Care, should those services be needed. 

  • Physician communication.

    GLHH works under the direction of your physician. Your physician will always know what is happening by the home health team because we regularly communicate this to them.

  • Seamless transitions.

    If your admission to home health is due to a discharge from a hospital or rehab center, we work closely with discharge planners and your primary care physician to ensure a seamless transfer of care takes place.

  • Family disclosure.

    In situations where family members have requested and been authorized to receive updates on the care, the home health team is delivering, our team of clinicians and case managers will communicate with the family on a regular basis to ensure everyone is aware of the current status of care.

  • Keeping up to date.

    Many of our patients have not only their primary care physician but also specialty physicians such as a cardiologist, nephrologist, pain management physician, etc. GLHH acts as your advocate ensuring that all the relevant physicians are kept up to date with the care being delivered and ensuring that none of the physicians are accidentally prescribing medications or treatments that may adversely interact with each other.

  • Changes in medication.

    During transitions of health status, it is not uncommon for the primary care physician to change medication regimes. In these instances, the home health team will ensure new medications are well understood by the patient and will also work closely with the pharmacy to ensure that the medication prescriptions will be properly filled.
